I hereby give permission for my son/daughter to participate in the RLX RedBlacks Lacrosse Club Clinics, Summer Lacrosse Camp, Hype Lax and/or our Field and Box Lacrosse travel team tryouts, practices and tournaments.
I understand the physical risks involved with the participation in lacrosse, and hereby release the RLX RedBlacks Lacrosse and all clinic and coach staff from responsibility for injury that may occur. I approve of my child’s participation, and verify that he/she is in good health.
I certify that my child is in good physical and mental health and in case of emergency, when not present, I give permission for RLX RedBlacks Lacrosse staff and/or management to seek proper medical care for my child.
I agree that RLX RedBlacks Lacrosse may use my child's photograph and name in any and all promotional material that RLX Lacrosse may employ, including but not limited to social media (twitter, Instagram, YouTube, etc), website, printed newspapers and magazines.
I give permission for RLX RedBlacks Lacrosse to continue to send me, the child's parent/guardian, updates about the programs RLX Lacrosse currently offers and will offer in the future
By signing below, you agree that you are the parent/guardian of the player being registered and have full legal responsibility for the decisions of said player, and that you have read this Agreement and agree to be bound by this Agreement.
Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal and provincial governments and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.
RLX RedBlacks Lacrosse has put in place preventative measures to reduce the spread of COVID-19; however, the RLX cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the clinics could increase your risk and your child(ren)’s risk of contracting COVID-19.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the clinics and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the clinics may result from the actions, omissions, or negligence of myself and others, including, but not limited to, clinic employees, volunteers, and program participants and their families.
I also declare that my child (over the age of 12) has had 2 vaccine doses, as these are mandatory for entering into any facility that the RedBlacks are using for both their indoor and outdoor events.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the clinic . On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold RLX RedBlacks lacrosse, its coaches, management, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of RLX RedBlacks lacrosse, its coaches, management, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any RLX RedBlacks lacrosse program.
